This will kill her instantly.

She will be unrecruitable, and pitiable.

Alternatively, ignoring the quest entirely will also result in Minthara’s death.

If you choose to romance her, you will get the option to go with her.

If she was not romanced, she returns at Withers' epilogue party having begun her rebellion.
